Starting October 1, 2024, you will need to verify your income, name, address, and the number of people in your household to continue receiving food from The Emergency Food Assistance Program (TEFAP). This requires completing form F-40059.
If you wish to access additional food from the pantry or if your household income exceeds the eligibility level, you must also provide:
Note that gift cards and other local programs are only available to residents of the Oregon School District. We will verify that your address is within the OSD boundaries.
If your information changes throughout the year (e.g., address, household members, income), you must notify us to update our records and confirm your eligibility.
